Media Release Form
I, the undersigned, hereby authorize the Wassmuth Center for Human Rights (the “Center”) and each of its respective representatives to take photographs, video/audio recordings and interviews (“Depictions”) of me and/or the minor child registered for Kindness Camp (“Minor Child”) for the Center’s lawful use in connection with their programs, including use in any audio or visual work, marketing materials, print campaigns, posters, commercials, social media, internet sites or any other media (whether now known or hereafter devised) (the “Works”). I hereby assign fully to the Center all right, title and interest in and to all Depictions, and accordingly grant the Center and the Filmmakers the right to reproduce, exhibit, display, broadcast and distribute the Works or Depictions, and all derivative works thereof.
I hereby release and hold harmless the Center, including each of its affiliates, directors, officers, employees, contractors, volunteers, representatives and agents from and against any and all losses, claims, penalties, demands, actions, causes of action, damages, complaints, or liability arising out of or related to the use of the Depictions of me and/or the Minor Child.
I, further waive any right to inspect or approve the use of the Depictions. I also waive any right that I and/or the Minor Child may have to any royalties or other compensation arising from or related to the use of the Depictions or Works, including reimbursement for any expenses incurred by me and/or the Minor Child in connection with participating in the above activities or the use of the Depictions as described above.
I hereby certify that I am the parent or legal guardian of the Minor Child registered for Kindness Camp. I have read this Media Release Form and fully understand its contents, meaning and impact and sign it of my own free act and will.
